FORMS AND REPORTS
Below is a list of various forms and reports that are used for the Premier Cricket season. To select a particular form or report, please click the appropriate link:
1. Premier Cricket Team Sheet
This form may be used by scorers to collect team information on match-days. A step-by-step guide on how to enter a team-sheet correctly is also available to download below.

2. Captain's Report
This form is available on MyCricket and has to be completed by all captains in the:
- West End 1st, 2nd, 3rd, 4th Grades
- West End One-Day Cup
- West End Twenty20 Cup
- Statewide Super Women's 1st and 2nd Grade
- Statewide Super Women's T20 1st and 2nd Grade
To be completed by all captains/team managers in the:
- U16 Red/White Grades
- U14 Red/White Grades
- Ray Sutton Shield

3. Venue Assessment Form - To be completed by 1st and 2nd Grade captains and coaches in the Men's and Women's competitions. Click here
4. Official Report by an Umpire for Breaches
This report is to be completed by an umpire for any breach of laws or bylaws during a Premier Cricket match

6. Post-Match Meeting Report - Umpire Performance Report Form
This report is to be completed by a facilitator, umpire and both captains after the completion of a match. This form can be used to evaluate general match issues, player and umpire performance as well as the pitch and outfield.

7. Permit Request Forms
All Permit request forms must be submitted to the SACA Premier Cricket Coordinator 48 hours prior to the start of a Premier Cricket Match.
A Permit for Replacement Players Form must be completed to provide information on any players that will be replaced in a Premier Cricket match.
8. Major Rounds - Permit Request Forms
All Permit request forms must be completely filled and submitted to the SACA Premier Cricket Coordinator by 5.00pm on the Wednesday prior to the start of the final round, for which the permit is being applied for. Any applications received after 5.00pm will not be considered.
Junior Grades - A Permit to Play Request Form must be completed for a Junior Grade player that is nominated into a Major Round team but does not meet the eligibility requirements for a Major round match.
Women's Grades - A Permit to Play Request Form must be completed for a Women's Grade player that is nominated into a Major Round team but does not meet the eligibility requirements for a Major round match.
Men's Grades - A Permit to Play Request Form must be completed for a Men's Grade player that is nominated into a Major Round team but does not meet the eligibility requirements for a Major round match.
9.Premier Cricket Player Registration Application
Premier Cricket Player Registration form must be completed and signed by all respective parties.
